📋 Description

• Evaluate and assess patients’ healthcare needs using established triage protocols and guidelines • Participate in weekly huddles to review KPIs and ensure workflows meet expectations, including call-time limits • Obtain provider input to determine appropriate clinical actions and recommendations • Document symptoms, nursing assessments, advice, responses, plans, follow-up, and implementation in the EHR • Follow policies, procedures, protocols, standing orders, and triage algorithms • Coordinate and schedule same-day and future provider appointments • Manage prescription refills per protocol • Participate in CPC+ Care Coordination team processes • Provide patient education on chronic disease, preventive care, and family health • Participate in quality and performance improvement activities • Forward pertinent medical information to appropriate departments for continuity of care • Role-model Just Culture and organizational values

🎯 Requirements

• 3 years clinical experience with applicable patient care and administrative duties preferred • Knowledge of disease processes, normal growth and development of all age groups required • Nursing assessment skills with/without visual inspection utilizing basic nursing theory required • Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required • State of Colorado Registered Nurse or valid compact state license required • Must be able to live and work primarily in one of the listed states: Arizona, Colorado, Connecticut, Florida, Georgia, Idaho, Illinois, Kansas, Massachusetts, Maryland, Michigan, North Carolina, New Jersey, Ohio, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Virginia, Washington, or Wisconsin • Must be HIPAA compliant
