• Serve individuals with serious mental illness (SMI) who are being released from a DAC Facility • Ensure communication between all parties • Provide case management to individuals referred in the process • Ensure referrals are made to necessary services including medication management, housing support, and more.
• Master’s degree in Psychology, Social Work, Counseling, or similar field • At least two years of progressive experience providing similar services to the populations served (MH/SU/IDD) • Must be fully or provisionally licensed mental health clinician.
