📋 Description

• Conduct in-reach, engagement, education, and support for adults with serious mental illness or serious and persistent mental illness • Inform members/recipients about community-based mental health services and supported housing options, including tenancy support services and rental assistance • Educate members/recipients about Medicaid, Special Assistance, North Carolina State Plan services, and state-funded services • Meet minimum daily, weekly, and monthly in-reach, transition, diversion, and informed decision-making requirements • Maintain the in-reach caseload and ensure each person receives a Peer Support Specialist visit before 90 days • Coordinate face-to-face meetings with members/recipients and guardians regarding integrated supported housing and behavioral health services • Assess interest in supported housing and explain its benefits and financial aspects • Facilitate and accompany members/recipients on site visits to permanent housing with tenancy rights • Address concerns of members/recipients who decline or are ambivalent about supported housing • Review housing preferences and provide linkage to ongoing in-reach and available housing supports • Document activities and track outcomes, including refusals and lack of response • Participate in North Carolina state-level training, information sessions, and committees related to the Transitions to Community Living Initiative • Coordinate with Transition Coordinators, care management, and other Vaya departments to support smooth transitions • Perform other duties as assigned

🎯 Requirements

• High school diploma or GED required • 1–2 years of experience working with the MH/SU population • NC Certified Peer Support Specialist certification required • Knowledge of recovery methods and person-centered thinking • Customer service skills • Ability to establish respectful relationships with people with disabilities and their families • Ability to interact professionally and effectively with upset or disagreeing individuals • Strong communication skills, orally and in writing • Knowledge of laws, regulations, and policies governing human services helpful • Knowledge of Medicaid, Special Assistance benefits, clinical services, community supports, and supported housing • Ability to work effectively as a team member and under the direction of the Peer Support Specialist Manager • Must reside in North Carolina or within 40 miles of the North Carolina border • Must comply with HIPAA, 42 CFR Part 2, and applicable confidentiality laws • Must complete required confidentiality training and sign a confidentiality statement • Must be able to perform required visual, physical, and sedentary work activities
