📋 Description

• Deliver personalized, evidence-based virtual coaching to help members achieve lasting behavior change, navigate healthcare resources, and meet personal wellness goals • Primarily support the multidisciplinary Weight Management program • Conduct 30-minute personalized 1-on-1 coaching sessions using motivational interviewing and evidence-based behavior change strategies • Surface intrinsic motivation and co-create actionable SMART goals • Facilitate group coaching sessions and workshops • Provide ongoing accountability, evaluate progress, and adapt strategies to drive long-term member success • Administer appropriate screening tools and health measurements within scope of practice • Maintain timely clinical documentation and provide customized care plans • Partner with care managers, registered dietitians, and providers within structured weight management programs • Serve as the subject matter expert in behavior change for the multidisciplinary care team • Use health conditions and trend data to guide members to appropriate care and eliminate barriers • Connect members with digital health tools, health plan benefits, employer wellness initiatives, and community resources • Resolve member concerns and conduct proactive outreach calls • Manage scheduling, appointment coordination, and follow-ups • Participate in wellness campaigns, incentive programs, and client events • Adhere to the NBHWC Code of Ethics, HIPAA, PHI/PII protection protocols, and IT security standards • Escalate clinical or technical concerns according to standard operating protocols • Participate in quality assurance reviews, call audits, and routine feedback • Perform additional duties as assigned

🎯 Requirements

• National Board-Certified Health and Wellness Coach (NBC-HWC) credential • Bachelor’s degree in Health Science, Kinesiology, Nutrition, Psychology, or a related field • 2+ years of health coaching experience, preferably within a clinical, healthcare, or digital health environment • Must be Bilingual English/Spanish or multilingual fluency • Experience with weight management • Ability to work Monday–Friday, 8 am–5 pm PST • Candidates who advance to the interview with the hiring manager will participate in a ~10-minute mock coaching session • Strong baseline knowledge distinguishing the clinical boundaries between health coaching, health education, and behavioral health/therapy • Proven customer service excellence with a track record of driving patient engagement and retention • Technical proficiency with Electronic Medical Record (EMR) systems, Salesforce, and Google Workspace preferred • Certified Diabetes Prevention Program (DPP) Lifestyle Coach preferred • Experience working with patients on GLP-1s preferred • Experience delivering care via telehealth or working within a remote team structure preferred • Exceptional verbal and written communication skills with a professional, empathetic demeanor • Strong caseload management, multitasking, and acute attention to detail • Agile and resilient in a fast-paced environment; open to evolving workflows and continuous feedback • Willingness to adjust working hours based on member, company, and client availability needs • Dedicated, private, and professional workspace for virtual visits • Prolonged periods of sitting or standing at a desk • Extensive daily time spent using a computer, headset, phone, and video conferencing tools • Manual dexterity for typing and computer entry; corrected vision, hearing, and speech within normal ranges
