S. nurse-led clinical support. It partners with physician practices, hospitals, ACOs, and other providers to improve patient engagement, clinical outcomes, workflow efficiency, and revenue through real-time data, device integration, and billing/reimbursement support.

📋 Description

• Build trusted relationships with patients using motivational communication techniques • Triage physiological markers and use the data to promote positive lifestyle and health behavior changes • Collaborate with patients’ care teams to provide wellness calls • Outline patient-centric goals and develop associated action plans • Monitor blood pressure, weight, and blood glucose with clinical appropriateness • Meet team goals, standards, metrics, and patient engagement program goals • Drive longitudinal patient engagement in remote patient monitoring programs • Provide preventive health and disease management education and coaching • Perform monthly wellness calls with assigned patients • Lead collaborative wellness calls to define health goals outlined by the care team • Manage patient messaging and alerts • Direct patients to treating physicians for routine questions • Follow appropriate escalation pathways for urgent care needs • Perform synchronous and asynchronous patient communication through Vivo Care’s Remote Care Platform • Proactively monitor vital signs to reduce risk of emergent care and hospital admission/readmission

🎯 Requirements

• Unrestricted RN/LPN/LVN license in a compact state and Massachusetts • Ability to work EST zone between 8:00a and 5:00pm • 1–3 years of clinical experience, patient management, or disease management desired • Experience working with different provider practices and workflows • Ability to work independently with minimal direction • Experience with Medicare patients • Experience performing virtual visits with patients and telephonic care management • Clinical background with adult and geriatric patient populations • Experience with phone triage ideally • Working knowledge of remote-patient monitoring (RPM) preferred and/or Chronic Care Management (CCM), Behavioral Health, Care Coordination, or Utilization Management principles • Experience with remote working technologies • Motivational Interviewing/Health Behavior Change experience a plus • Health Coach certification a plus • Spanish speaking a plus • Interest in professional leadership growth and development opportunities • Interest in operating in a new clinical program and contributing to processes and best practices
