📋 Description

• Build trusting relationships with patients using motivational communication techniques • Triage vital signs and use data to promote positive lifestyle and health behavior changes • Collaborate with patients’ care teams to establish patient-centric goals and action plans • Proactively monitor remotely transmitted blood pressure, weight, and blood glucose data • Provide preventive health and disease management education and coaching • Perform monthly wellness calls with assigned patients • Lead collaborative wellness calls to define health goals outlined by the Care Team • Manage patient messaging and alerts • Direct patients to treating physicians for routine questions • Follow escalation pathways for urgent care needs • Perform synchronous and asynchronous patient communication through Vivo Care’s Remote Care Platform • Meet team and patient engagement program goals and reimbursement requirements

🎯 Requirements

• Unrestricted RN/LPN/LVN license in a compact state and New York • Ability to work EST zone between 8:00a - 5:00pm • 1-3 years of clinical experience, patient management, or disease management desired • Experience working with different provider practices and workflows • Ability to learn quickly • Ability to work independently with minimal direction • Experience with Medicare patients • Experience performing virtual visits with patients and telephonic care management • Clinical background working with adult and geriatric patient populations • Spanish speaking a plus • Motivational Interviewing/Health Behavior Change experience a plus • Health Coach certification a plus
