• Assist ECM Sales/Marketing and Clinical Outreach Specialists teams with organizing and enrolling patients in programs • Facilitate the patient enrollment process by establishing positive relationships with teams, patients, and families • Conduct direct outreach to potential clients to educate them about offerings and facilitate enrollment • Facilitate intake and admission for incoming patients, with outreach assistance as needed • Provide accurate and ongoing assessment of patient status during admission • Maintain databases, logs, census sheets, resource/contact lists, schedules, and department information sheets • Complete insurance eligibility checks and assign clients to the Clinical Outreach Specialist • Work cross-functionally with sales, outreach, care delivery, and RCM to facilitate enrollment workflow • Record all enrollment-process interactions in the operating system • Participate in staff, census, and other meetings, including weekly Outreach Team meetings • Meet or exceed established departmental KPIs • Answer incoming calls and emails • Assume other duties supporting organizational needs
• Minimum of 2 years of previous intake or admission experience in a high call volume environment • CMA/CCMA certificate issued by a recognized group; active certificate required by the application • Prior experience in a remote role • Quiet, secure workspace to ensure member privacy • 2+ years of experience in palliative care, home health, hospice, and/or hospital discharge planning or case management highly preferred • 2+ years of experience working within or directly with one or more large safety net hospitals in the region preferred • Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time, with the possibility of evenings and weekends • Strong understanding of cultural competency with the target population • Excellent oral and written communication skills • General computer skills and working knowledge of the internet • Ability to independently learn and use Google Suite and Salesforce • Training as or alongside social workers, nurses, medical assistants, chaplains, medical scribes, or other educational or work experiences demonstrating fluency in clinical language and settings • Project development skills and ability to work independently • Bilingual (English/Spanish) highly preferred • Must pass background screening, which may include a drug test or other health screenings • Patient-facing employees must be vaccinated against influenza; religious or medical accommodations may be considered • Must comply with identity and work eligibility verification using E-Verify upon hire • Must be legally authorized to work in the United States and indicate whether employment visa sponsorship will be required
