📋 Description

• Manage patients assigned to the Care Team and align RN and Interdisciplinary Team visit frequencies with patient acuity and needs • Communicate with patients via phone and video conferencing according to care plans • Provide patient care within scope of practice and provider orders • Assess and enroll patients and complete initial referral follow-ups • Develop and update care plans based on ongoing assessments • Manage patient medications, including implementing orders, maintaining medication lists, assisting with refills, and ensuring adequate supply • Provide patient follow-up and education on new medication orders • Facilitate requisition of medical supplies and durable medical equipment • Complete care coordination, documentation updates, and communication with primary care providers and other physicians • Coordinate with the Interdisciplinary Team and external agencies, including advanced care planning and hospice referrals • Educate patients and caregivers on Vynca services, disease processes, symptom management, and medication tracking • Cover patient care for other care teams during normal working hours • Perform additional duties as assigned

🎯 Requirements

• Active, unrestricted California Registered Nurse (RN) license • Ability and willingness to obtain licensure in other states • 1+ year of experience in palliative or hospice care • 1+ year of experience as a professional nurse within the previous three years • Excellent oral and written communication skills • General computer skills • Working knowledge of MS Office and the internet • Willing and able to work Monday through Friday 8:30am–5:00pm Pacific Time, with flexibility for potential evenings and weekends • Oregon RN license highly preferred • CHPN certification or ability to obtain ACHPN certification • Remote work experience • Legal authorization to work in the United States • Must not require employment visa sponsorship
