âą Manage clinical programs day to day, including maintaining databases and processes to schedule and support patients and deliver against program design âą Assist with clinical disease management through care plans âą Conduct telephonic patient assessments and health coaching for chronic diseases âą Support patient and prescriber engagement for clinical program initiatives âą Assist with clinical and operational demands of other business units as needed âą Educate providers on claims and benefit procedures âą Develop reports supporting work and outcomes for patients and clients âą Review literature and formulate comprehensive responses to drug information questions or needs âą Assist with establishing policies, procedures, and internal processes âą Conduct drug utilization reviews and interventions âą Help develop and operationalize clinical programs under the guidance of the Manager of Clinical Programs âą Conduct comprehensive medication reviews and patient consultations on drug therapy âą Collaborate with clinical program staff, including nurses and health coaches, to optimize population management and coordination of care âą Communicate with patients and providers, including outbound calls âą Comply with Federal and State pharmacy rules and regulations and company policies and procedures âą Report directly to the Manager, Clinical Programs
âą Doctor of Pharmacy degree obtained from an accredited college or university âą Active pharmacist license in good standing in state of practice âą Active/Clear License to practice in Florida as required by profession âą Proficiency with Microsoft Word, Excel, and Outlook required âą Two years minimum clinical experience preferred âą 2-3 years of experience in a managed care setting preferred âą Experience in providing clinical presentations and/or progress updates preferred âą Experience in formalized projects or improving clinical operational processes preferred âą Exposure to clinical and analytical problem-solving and expertise in drug information âą Proficiency with Power Point and Access preferred âą Ability to conduct presentations to internal departments or external clients on clinical program initiatives âą Knowledge of scope of clinical services allowed by the profession and applicable laws and regulations âą Ability to evaluate drug therapy appropriateness, effectiveness, safety, and adherence âą Ability to communicate with healthcare providers and ensure continuity of care âą Must be dependable and maintain confidentiality
