📋 Description

• Conduct virtual medical evaluations via video, phone, and asynchronous forms, confirming diagnoses, assessing target organ damage, and documenting baseline metrics • Create and manage holistic, evidence-based care plans for obesity, dyslipidemia, hypertension, ASCVD, mild-to-moderate CKD, and pre/diabetes • Act as primary prescriber in Full Practice Authority states, including medication titration and renewal according to clinical protocol • Prescribe under Collaborative Practice Agreements with the Medical Director in non-Full Practice Authority states • Manage device-triggered Red Alerts, triaging patients for ER referral or immediate titration adjustment during business hours • Provide warm handoffs to referring clinicians in urgent situations, including new renal failure presentations • Document all synchronous and asynchronous patient interactions in the EHR with accurate ICD-10 coding • Accurately capture comorbidities to ensure correct reimbursement • Prepare PCP care coordination reports for referring physicians • Collaborate with the Medical Director to develop and refine evidence-based titration algorithms and clinical protocols • Build scalable workflows to support rapid patient volume growth • Provide feedback on app-based care tools, behavioral support content, and remote monitoring integration

🎯 Requirements

• Active NP license in good standing in the candidate’s home state • IMLC/eNLC compact license strongly preferred • Willingness to obtain licensure in target states within 90 days of start • Board certification through ANCC or AANP as a Family, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care NP • Minimum 2 years of post-licensure clinical experience in primary care, internal medicine, endocrinology, cardiology, or geriatrics • Strong command of cardiometabolic conditions • Comfort with telehealth or digital care delivery in a screen-based, asynchronous-forward environment • Strong documentation discipline and familiarity with EHR systems • Excellent independent clinical judgment and ability to make decisions without a physician in the room • US residency, including all 50 states and US territories • Experience with value-based care, CMS quality measures, ACO/care management programs, CKM conditions, GLP-1 therapy management, Collaborative Practice Agreements, clinical program building, or Spanish proficiency are bonus qualifications
