📋 Description

• Provide complex, high-risk care management for pediatric members • Conduct assessments and screenings • Develop ongoing care plans and service plans • Evaluate member needs, barriers to care, and social determinants of health • Collaborate with providers, specialists, caregivers, and community resources • Identify problems and barriers to care and provide interventions • Coordinate person-centered care and services • Follow up and monitor member status, condition changes, and care-plan progress • Provide resource support for services including employment, housing, independent living, justice, and foster care • Facilitate timely access to needed care and services • Perform telephonic, digital, home, and/or other site outreach • Collect, document, and maintain member information and care-management activities for regulatory compliance • Educate members and families/caregivers on disease processes, care gaps, care options, referrals, and healthcare benefits • Support other Care Managers by answering questions and providing guidance on complex cases • Provide feedback to leadership on opportunities to improve care and quality delivery • Perform other assigned duties and comply with policies and standards

🎯 Requirements

• Experience in pediatric care management • Active Registered Nurse (RN) license • Candidates must reside in North Carolina • Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing • 2–4 years of related experience • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
