📋 Description

• Manage care for members with complex, high-risk needs • Conduct assessments and screenings • Develop, assess, and facilitate complex care management activities • Develop personalized care plans and provide education for members and families • Evaluate member needs, care-access barriers, and social determinants of health • Collaborate with providers, specialists, caregivers, and community resources • Identify problems and barriers to care and provide care management interventions • Coordinate person-centered care and services • Follow up and monitor member status, condition changes, and care-plan progress • Provide resource support for members and Care Managers • Facilitate timely access to needed care and services • Conduct telephonic, digital, home, and other site outreach • Collect, document, and maintain member information and care-management activities • Educate members and families on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits • Support other Care Managers by answering questions and providing guidance on complex cases • Provide leadership feedback on opportunities to improve care quality and cost-effectiveness • Perform other assigned duties and comply with policies and standards

🎯 Requirements

• Must have maternity care management experience • Must live in North Carolina • Experience working with the pediatric population is required • Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing • 2–4 years of related experience • RN - Registered Nurse - State Licensure and/or Compact State Licensure required • Compliance with all policies and standards
