📋 Description
• Collaborate with Market President to deliver results on KPIs and overall P&L outcomes • Upskill APPs through high-intensity provider coaching based upon KPI performance • Manage highest need, highest risk patients in short intervals to stabilize, then transition to APP panels as appropriate • Lead health plan and network provider partnership discussions • Drive relationship management of partners to key business results including growth targets, partner engagement and clinical/financial outcomes • Support board material content and creation • Other duties as assigned
🎯 Requirements
• Must live in Florida • M.D. or D.O. Graduate • Board Certification in Family Medicine or Internal Medicine Required • Fellowship training in Geriatrics other professional degrees (MPH, MBA, JD) welcome but not required • 5-15 years experience in outpatient or home-based practices, value based care experience preferred • Extensive experience in clinical leadership roles coaching and developing NPs, PAs or MDs • Experience utilizing KPIs and data to drive improved cost, quality and clinical outcomes for patients • Comfort with ambiguity; experience in a start-up environment is helpful
