📋 Description

• Serve as a primary point of contact for patients enrolled in the Patient Support Program • Provide education regarding prescribed therapies, treatment expectations, medication administration, and adherence strategies • Support patient enrollment and onboarding activities • Coordinate care and communicate with physicians, clinic staff, specialty pharmacies, and other healthcare professionals • Assist patients in navigating access to treatment and reimbursement processes • Conduct patient assessments and follow-up activities as required by the program • Monitor patient progress and provide ongoing support throughout the treatment journey • Maintain accurate and timely documentation within program systems • Identify and escalate clinical or operational concerns as appropriate • Contribute to quality improvement initiatives and program success measures • Build and maintain positive working relationships with healthcare providers and clinic partners • Travel to clinics, healthcare facilities, and stakeholder meetings throughout Alberta and British Columbia

🎯 Requirements

• Active and unrestricted nursing license in Alberta and/or British Columbia (RN, RPN/LPN, or equivalent nursing designation) • Diploma, degree, or equivalent combination of education and experience consistent with the P1 Professional Career Framework • Strong verbal and written communication skills • Proficiency using Microsoft Office and electronic documentation systems • Ability to work approximately 2.5 days per week within program operating hours of 8:00 a.m. to 8:00 p.m. EST, Monday through Friday • Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia as required • Excellent organizational skills and attention to detail • Valid driver's license and access to reliable transportation may be required based on territory coverage • Ability to work independently in a remote environment while effectively managing competing priorities • Occasional lifting and transportation of educational or program materials may be required
