📋 Description

• Manage high-volume inbound and outbound telephonic patient interactions in a centralized clinical call center • Conduct 70–90 outbound calls per day to check in on patients and address health concerns • Handle inbound calls, triaging and routing them according to clinical urgency • Route non-clinical calls to appropriate company departments • Monitor and respond to Remote Patient Monitoring alerts in real time, escalating clinically indicated concerns • Collaborate with providers to coordinate timely patient care by telephone • Perform monthly wellness assessments and comprehensive chart reviews • Document all patient interactions in the clinical platform in real time • Meet or exceed care quality, patient engagement, call handle time, response time, and protocol adherence metrics • Pivot among RPM adherence calls, onboarding calls, and chart reviews to support departmental goals • Maintain compliance with company policies and applicable regulations • Perform other duties as assigned

🎯 Requirements

• Active compact LVN/LPN license required • Proficient with computers, EMRs, and telehealth tools • Strong communication and organizational skills • Demonstrated AI fluency; actively uses tools such as Claude, ChatGPT, or workflow automation in daily work • Ability to work in a fast-paced, high-volume call center environment with back-to-back patient interactions • Comfort with performance-based metrics and accountability to daily, weekly, and monthly targets • Ability to multitask across multiple technology platforms simultaneously • Prior call center or high-volume telephonic care experience • Familiarity with RPM devices, including blood pressure cuffs, glucometers, pulse oximeters, and weight scales • Minimum internet speed of 50 Mbps download / 10 Mbps upload • Hardwired internet connection required; no Wi-Fi • Speed test submission required during the offer process • Private, HIPAA-compliant workspace • Must be available for rotating holidays • Mandatory coverage of a minimum of two weekends per month • Four-week training attendance is mandatory, Monday through Friday, 9:00 AM–6:00 PM Eastern Time • At least 1 year of nursing experience preferred; RPM, telehealth, or chronic care experience is a plus
