📋 Description

• Complete remote order entry, order verification, and drug verification • Perform transitions-of-care medication reconciliation • Navigate multiple pharmacy information systems • Provide drug information and therapeutic guidance to providers • Assess medication appropriateness, dosing including renal/hepatic adjustments, interactions, and compatibilities • Support pediatrics, geriatrics, pregnant/nursing patients, HIV patients, and patients with renal/hepatic impairment • Apply knowledge of IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, chemotherapy, and other therapies • Document interventions and clinical actions accurately and promptly • Communicate with physicians and nurses • Ensure compliance with HIPAA, pharmacy laws, and client policies • Participate in ongoing training • Meet productivity and quality standards

🎯 Requirements

• Bachelor's in Pharmacy required; PharmD preferred • At least 1 year of hospital pharmacy experience strongly preferred • Active and in-good-standing pharmacist license in Illinois or California required • Active pharmacist licenses in Illinois, Nebraska, Hawaii, Iowa, and California required per job title/licensure requirements • Willingness to obtain additional state licenses (Iowa, Nebraska, Hawaii) • Strong clinical knowledge and comfort working across multiple systems • Ability to work evenings, overnights, weekends, and holidays • Ability to work remotely with cable broadband or fiber optic internet service of at least 10 Mbps download/5 Mbps upload • Ability to work across VPNs, automation systems, and multiple pharmacy information systems • Proficiency with Google Suite/Microsoft Office and Snagit • Maintain required licenses, credentials, and system access
