📋 Description

• Manage pediatric, palliative and/or acute medically complex clients with COPD or CHF • Direct Personal Support Workers (PSWs) to provide proper care • Provide remote assessment and management of care services to clients • Maintain accurate client clinical records • Collaborate with multi-disciplinary health care team

🎯 Requirements

• Registered and in good standing with the College of Nurses of Ontario • CNA Certificate in Palliative Care or Fundamentals of Palliative Care and Comprehensive Advanced Palliative Care Education (CAPCE) • Extensive hands-on experience in Pediatric Care • Recent experience with COPD and CHF patients required • Digital and electronic health experience, EMR and mobile technology • Valid CPR C / First Aid required • Access to a personal vehicle for business use and possess a current driver’s license and proof of vehicle insurance • Current and original copy of a satisfactory Criminal Records Check is required
