📋 Description

• Processing and entering referrals into Axxess EMR • Scheduling clinicians appropriately based on discipline, availability, and payer requirements • Obtaining and verifying home health insurance authorizations • Reviewing and processing physician orders • Faxing and following up on orders with physicians • Uploading and maintaining documentation and orders in Axxess • Managing intake workflow from referral through admission and scheduling • Understanding Medicare home health guidelines, including OASIS requirements and compliance expectations • Working efficiently in a fast-paced, high-volume environment with minimal supervision

🎯 Requirements

• Highly organized and detail-oriented • Experienced in home health intake or coordination roles • Able to work independently with strong follow-through • Comfortable managing multiple priorities simultaneously • Familiar with Medicare Conditions of Participation and home health compliance workflows • Strong in communication with clinicians, physicians, and internal staff
