📋 Description
• Maintain ongoing communication with patients enrolled in the PCM program • Monitor patient conditions, including: Symptoms, Medication adherence, Progress and response to treatment • Provide patient education regarding their condition and care plan • Assist in the implementation and follow-up of care plans • Coordinate services and ensure continuity of care • Identify changes in patient status and escalate concerns to the provider when needed • Ensure patients follow treatment recommendations and scheduled appointments • Accurately document all patient interactions and updates in the system
🎯 Requirements
• Licensed Nurse or MD (required) • Advanced English level (C1 or higher) – excellent spoken and written communication • Proven experience working with U.S.-based medical offices • Experience with chronic care management, patient follow-up, or care coordination • Strong clinical judgment and ability to assess patient conditions remotely • Experience providing patient education and support • Highly organized with strong documentation skills • Comfortable working independently in a remote environment • Reliable computer (8GB RAM minimum) • Stable internet connection (minimum 100 Mbps) • Quiet and professional workspace • Headset with microphone
