📋 Description

• Act as the central communication hub between patients, physicians, specialists, and multidisciplinary care teams • Build, update, and monitor individual patient care plans • Track progress against clinical goals and schedule follow-up appointments • Maintain accurate electronic health records (EHR/EMR) • Process referrals and perform insurance eligibility verification • Handle patient inquiries via phone, email, or chat • Connect patients with community resources, home care services, medical equipment providers, and financial support programs • Conduct patient check-ins to monitor treatment adherence and address care barriers • Educate patients on prescribed care plans • Ensure coordination activities comply with HIPAA, patient privacy laws, and SOPs • Identify treatment delays, care gaps, and recurring operational issues • Escalate issues to clinical leadership for resolution

🎯 Requirements

• 3+ years of experience in US healthcare coordination, medical administrative support, patient navigation, or case management • Proficiency with Electronic Health Record (EHR) systems • Proficiency with HIPAA-compliant communication tools • Proficiency with Microsoft Office Suite and Google Workspace • Exceptional verbal and written communication skills • Strong empathetic listening abilities • Outstanding time-management skills • High attention to detail • Ability to manage multiple care tracks simultaneously • Preferred: prior experience in a remote healthcare or telehealth environment • Preferred: insurance verification and prior authorization knowledge • Bilingual capabilities as needed by the patient demographic • High empathy and patient-centric focus • Strong problem-solving and conflict-resolution abilities • Proactive follow-through and accountability • Adaptability in fast-paced healthcare settings
