Beschreibung

• Serve as a role model for individuals facing substance-exposed living and those in various phases of recovery • Follow Substance Use Disorder Program guidelines to plan care, assess client needs, and complete screenings • Refer clients to appropriate resources • Coordinate with the Substance Use Disorder Program team, primary care providers, managed care organizations, and community agencies • Identify individual member needs and document interventions based on relationships and assessments • Provide member support in a variety of settings and assist with support groups as needed • Contribute to case management meetings • Complete documentation for member contacts, touches, cancellations, and care plans • Communicate professionally with members, families, physicians, community agencies, agency leadership, and office staff • Follow chain of command when communicating concerns or recommendations • Achieve set goals and Key Performance Indicators (KPIs) • Represent and promote Mindoula and the Substance Use Disorder program through community and professional organization participation and outreach • Participate in conflict resolution and performance improvement processes • Support team members in performing their duties • Promote responsible and ethical stewardship of Mindoula's resources • Maintain punctuality and attendance during work hours • Partner with the enrollment team to grow member census and identify eligible members for enrollment • Perform other duties as assigned

🎯 Anforderungen

• Fully certified as a Peer Support Specialist in WA • Washington Certified Peer Recovery/Support Specialist • Successfully participated in a recovery program • Strong organizational skills • Problem-solving skills • Ability to work primarily from home with occasional community-based work as needed
