📋 Description

• Conduct wellness consultations, including Social Determinants of Health screenings, health coaching, health education, and referrals • Promote preventative care and health screenings • Conduct SDOH screenings to identify individual or family needs • Refer clients to community resources and agencies • Conduct community outreach to promote, maintain, and improve health • Help clients navigate community services and adopt healthy behaviors • Provide chronic-disease prevention education • Provide vital sign intake, HIV testing, lead testing, and point-of-care testing • Document client engagement details and conduct follow-up contacts • Assess eligibility for financial assistance, insurance, and other programs • Help clients obtain healthcare services, including Medicaid, family planning, and medical-home connections • Serve as a liaison among communities, individuals, and service providers • Recruit new clients and identify referral partners • Strengthen partnerships with existing referral agencies • Participate in continuing education, community meetings, and health fairs • Operate office equipment and required software • Perform other assigned duties

🎯 Requirements

• High school diploma or GED equivalent • Level I: 6 months to 2 years of experience as a Community Health Worker • Level II: At least 2 years of experience as a Community Health Worker • Candidate must feel comfortable interacting with active and recovering drug users, recently incarcerated individuals, and unhoused individuals • Comfortable working in treatment settings, including inpatient and detox facilities • Knowledge of Social Determinants of Health and their impact on overall well-being • Knowledge of case management and/or care coordination • Knowledge of local health-promotion resources and agency programs • Knowledge of community outreach, healthcare, managed care, public/community health, or health promotion/education agencies • Knowledge of health coaching and health education • Skill operating standard office equipment and required software applications, including Microsoft Office • Ability to maintain confidentiality and follow HIPAA standards • Ability to work independently, solve problems, prioritize tasks, and adapt to complex situations • Strong oral and written communication skills • Ability to build rapport and trust with diverse populations • Valid driver's license and dependable transportation • Level I: Must complete 12-month apprenticeship program once CHWA certified • Level II: MiCHWA training certificate
