âą Support Quality, Safety, and Compliance âą Help identify recurring documentation and workflow opportunities for improvement âą Support quality assurance activities by ensuring clinical work meets organizational standards âą Order Management & EMR Workflow Support âą Review and process physician orders within the EMR âą Ensure verbal orders meet documentation requirements âą Confirm orders are complete and aligned with patient care needs âą Support intake and referral processing through clinical review as needed âą End of Episode Management âą Review discharge reports to ensure required documentation is complete âą Process and lock transfer to inpatient facility and death at home OASIS visits âą Verify discharge summaries and signed orders are present âą Confirm NOMNC documentation is completed when applicable âą Process discharge-related orders as needed âą Hospital Hold Process Support âą Manage hospital hold documentation and inpatient transfer workflows âą Assist with plan of care updates for hospitalized patients âą Support resumption of care workflows and monitor pending approvals âą Complete discharge workflows for patients who remain in facilities at the end of an episode when appropriate âą Quality & Compliance Monitoring âą Review infection control reports and escalate concerns as needed âą Monitor wound score documentation and identify variances âą Review quality improvement events and communicate trends to leadership âą Process clinical alerts and potentially avoidable event notifications âą Audit missed visits and related documentation to support quality care delivery and reimbursement compliance âą Monitor for timely initiation of care concerns âą Patient Status Monitoring âą Review vital sign alerts and ensure appropriate follow-up documentation is completed âą Monitor coordination notes for physician updates and clinically significant medication-related concerns âą Review EMR dashboards for changes in patient condition and escalate issues appropriately
âą Active RN license âą Prior home health field experience âą Minimum of 1 year of experience working in Homecare Homebase (HCHB) back office workflows âą Strong understanding of home health documentation and regulatory requirements âą Strong organizational skills and attention to detail âą Ability to manage multiple priorities in a remote work environment âą Effective written and verbal communication skills âą Multistate/Compact RN License preferred âą Willingness to seek additional state licensure âą Previous home health management experience preferred
