📋 Description

• Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests • Develop personalized treatment plans based on patients' needs and medical conditions • Prescribe nutraceuticals and/or medications and monitor their effectiveness • Collaborate with other healthcare professionals to coordinate patient care • Provide counseling and education to patients on disease prevention and healthy lifestyle choices • Maintain accurate and up-to-date patient records using electronic health record systems • Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine

🎯 Requirements

• NP/APRN – with a concentration in Functional Medicine, Hormone Health, Anti-Aging or Lifestyle Medicine • Formal training and experience practicing Bioidentical Hormone Replacement Therapy (BHRT) strongly preferred • Must have (or be able to obtain) an IL APRN and IL DEA licenses • Ideally full-time, but must be able to work at least 16-20 hours per week (including 1-2 evening shifts) • Strong oral and written communication skills • Energetic and outgoing personality with a desire to help people • Patient first attitude and ability to consistently provide a 5-star level of service • Knowledge of HIPAA regulations and ability to maintain patient confidentiality
