📋 Description

• Provide patient care in accordance with defined practice standards • Review all in-basket folders for assigned regional providers and promptly disposition messages and results • Provide clinical care for patients • Complete comprehensive patient assessments • Determine diagnoses and treatment of illness • Order and interpret medical diagnostics in tandem with the attending physician • Prescribe medications, treatments, therapy and medical equipment • Conduct patient education for chronic illness management or wellness care • Refer patients for specialty care when determined • Consult with physicians when needs fall outside Nurse Practitioner scope or when diagnosis/treatment is unclear • Participate in the healthcare team and promote communication with interdisciplinary staff • Act as a team resource • Work with the primary care team on program development • Liaise with outside agencies as needed • Perform other duties as assigned

🎯 Requirements

• Master's degree Nurse Practitioner/Physician Assistant program • 2 years experience in Primary Care • Licensed Registered Nurse - MN Board of Nursing required upon hire for NP, or Licensed Registered Nurse - WI Dept of Safety & Professional Services required upon hire for NP • NP or PA State of MN and/or WI license/cert required • BLS Tier 1 - Basic Life Support - Multisource required within 180 Days • DEA upon hire required • Ability to live in Minnesota or Wisconsin • Ability to lift up to 32 lbs. occasionally and up to 25 lbs. frequently
