📋 Description

• Manage AltaMed Managed Care participants assigned to the team • Work closely with providers and nurses to coordinate services and advocate for participants • Complete daily notes • Conduct family conferences • Assist with community referrals • Maintain regular contact with participants • Complete incident reporting

🎯 Requirements

• A bachelor’s or master’s degree in social work from an accredited university is required • Minimum of 2 years of experience in case management and social advocacy required • Experience working with the homeless population is preferred • Managed Care Experience preferred • Bilingual English/Spanish strongly preferred
