📋 Description

• Provide personalized, coordinated health care to long-term care residents enrolled in special Medicare Advantage plans • Assess patients’ medical and mental health needs and develop proactive plans of care • Treat patients with acute and chronic conditions using independent judgment, consulting a supervising physician as appropriate • Ensure compliance with local, state, and federal agencies related to clinical services • Prescribe medications and order lab work, diagnostic procedures, and consultations • Monitor treatment compliance and patient response, modifying care plans as needed • Collaborate with RN case managers in an integrated care team • Work with facility partners and health plan leadership to implement the national model of care • Provide training and advice to facility staff when required

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • Master’s degree or better • 3 years’ experience in clinical nursing or rehab in geriatric populations • Experience in primary care or internal medicine preferred, preferably with geriatrics focus • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Experience in primary care in geriatric medicine preferred • Long-term care experience • APRN Advanced Practice RN certification/license required
