📋 Description

• Conduct in-home infusion therapy visits according to the plan of care • Prepare clinical/nursing notes and document patient interventions • Act as a clinical resource and collaborate with pharmacy departments • Maintain knowledge of drug side effects and interactions • Provide telephone and in-person education on therapy protocols, drug side effects, and home infusion procedures • Ensure patients understand therapy plans, medications, and adherence requirements • Liaise between patients, families, and the pharmacy • Perform comprehensive initial and periodic head-to-toe patient assessments • Initiate preventative and rehabilitative care • Determine patient needs using assessment data and input from the care team, physician, patient, and family • Manage patient and family expectations regarding services, outcomes, and discharge goals • Establish primary and secondary diagnoses based on patient assessment and home health care focus • Develop care plans with skilled interventions, medical supplies, and necessary services • Evaluate patient progress and revise care plans as needed • Perform other duties as assigned

🎯 Requirements

• Graduate of an accredited nursing school • Current RN state license • 2+ years of experience in home infusion and infusion centers • 2+ years of experience with IV Immunoglobulins and IV Biologics • CRNI certification preferred • Experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients preferred • Effective interpersonal, time management, and organizational skills • Strong written and verbal communication skills • Ability to maintain consistently accurate records • Ability to direct information to the appropriate healthcare provider • Proficient computer skills, including word processing and internet/email use • Current CPR and BLS certification from Red Cross or AHA • Valid driver's license • Reliable transportation and automobile insurance • Ability to meet physical requirements, including lifting up to 20 pounds at times
