📋 Description

• Conduct in-home infusion therapy visits according to patients' plans of care • Prepare clinical/nursing notes and document patient interventions in clinical and non-clinical records • Act as a clinical resource and collaborate with pharmacy departments • Maintain knowledge of drug side effects and interactions • Provide telephone and in-person education on therapy protocols, drug side effects, and home infusion procedures • Ensure patients understand therapy plans, medications, and adherence requirements • Serve as liaison among patients, families, and the pharmacy • Perform comprehensive initial and periodic head-to-toe patient assessments • Determine patient needs using assessment data and input from the care team, physician, patient, and family • Manage patient and family expectations regarding services, outcomes, and discharge goals • Establish primary and secondary diagnoses based on assessment and home health care focus • Develop care plans with skilled interventions, medical supplies, and necessary services • Evaluate patient progress with team members and revise care plans as needed • Perform other duties as assigned

🎯 Requirements

• Graduate of an accredited nursing school • Current RN state license • 2+ years of experience in home infusion and infusion centers • 2+ years of experience with IV Immunoglobulins and IV Biologics • Effective interpersonal, time management, and organizational skills • Strong written and verbal communication skills • Ability to maintain consistently accurate records • Ability to direct information to the appropriate healthcare provider • Proficient in computer skills, including word processing and efficient use of the internet and email • Current CPR and BLS certification from the Red Cross and AHA • Valid driver’s license • Reliable transportation • Automobile insurance • Ability to provide infusion therapy and patient education to adult and pediatric patients • CRNI certification preferred • Experience with peripheral IV starts, infusion therapy, and phlebotomy preferred • Ability to lift up to 20 pounds at times
