📋 Description

• Provide advanced clinical pharmacy care, including counseling and ongoing pharmacotherapy management for patients with complex specialty conditions • Perform comprehensive medication therapy management (MTM) to identify drug interactions and optimize therapeutic outcomes • Develop and monitor individualized care plans, tracking laboratory values, clinical progression, and adverse drug events • Provide evidence-based clinical recommendations to healthcare providers regarding drug selection, dosing adjustments, and patient safety strategies • Document in the electronic health record (EHR) and specialty pharmacy software to meet accreditation standards • Collaborate across interprofessional teams to streamline prior authorizations, medication delivery logistics, and care transitions • Support multiple specialties in the Ascension Tennessee market, including Gastroenterology and Pulmonology in Murfreesboro and New Salem

🎯 Requirements

• Pharmacist credential obtained prior to hire date or job transfer date • Bachelor's or advanced degree in Pharmacy required • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution • Active, unrestricted State Pharmacist License or licensure by date of hire • ASHP-Accredited PGY-1 Pharmacy residency or three (3) years of relevant clinical pharmacy experience required • Active Tennessee license required by start date • Ability to provide clinical pharmacy care for complex specialty conditions • Knowledge of medication therapy management, drug interactions, therapeutic optimization, and patient safety • Ability to develop and monitor individualized care plans, including laboratory values, clinical progression, and adverse drug events • Ability to document in the electronic health record (EHR) and specialty pharmacy software • Ability to collaborate across interprofessional teams on prior authorizations, medication delivery logistics, and care transitions
