📋 Description

• Identify patients through health plan gaps-in-care lists, state immunization databases, patient medical records and a patient disease management registry • Collaborate with providers, review care needs and validate them against the medical record • Reach out to patients with validated gaps in care and facilitate meeting those needs • Support quality outcomes and high reliability in the Patient Centered Medical Home

🎯 Requirements

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required • Required professional licensure/certification can be used in lieu of education or experience, if applicable • BLS Provider preferred; American Heart Association or American Red Cross accepted
