ATC - South Jersey Locations is a telehealth care provider staffed by Nurse Practitioners who deliver remote clinical services to medically complex Medicaid and Dual Eligible patients. They conduct virtual patient assessments via video visits, manage medications, develop care plans, perform follow-up, and coordinate care with PCPs, specialists, managed care organizations, and community partners. The team leads advance care planning and goals-of-care discussions, supports HCC coding, risk adjustment, and quality gap closure for HEDIS and STAR measures through documentation, and participates in a shared rotating 24/7 after-hours triage service.

📋 Description

• Conduct virtual patient assessments through video visits • Provide medication management • Develop care plans and perform patient follow-up • Coordinate with PCPs, specialists, managed care organizations, and community partners • Lead advance care planning and goals-of-care conversations with patients and families • Support HCC coding, risk adjustment, and HEDIS and STAR quality gap closure through documentation • Participate in a shared rotating 24/7 after-hours triage component

🎯 Requirements

• Active Delaware NP license • Candidate must currently live in Delaware or surrounding states and be licensed in Delaware • FNP or AGNP board certification required; general NP certification is not sufficient • Minimum 3 to 5 years of independent NP practice • True primary care or longitudinal chronic disease management experience • Significant current and active telehealth experience • Comfortable making clinical decisions based solely on video visits without hands-on access, vitals, or remote monitoring equipment • Value-based care experience or demonstrated understanding of the philosophy • Familiarity with HCC coding, HEDIS measures, and STAR ratings in day-to-day practice • Active DEA and full prescriptive authority • Medicare and Medicaid provider number preferred • Entrepreneurial mindset and comfort with a startup environment • This must be the candidate's primary role • Candidates must not have multiple active employer commitments competing with a Monday through Friday, 8:00-to-5:00 schedule
