📋 Description

• Provide in-home health evaluations for health plan members • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed • Connect members with appropriate care and help identify gaps in care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Able to work safely and comfortably in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Able to travel locally and within licensed states as needed • Able to conduct virtual visits as needed • Able to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening • Availability for 40 hours per week
