📋 Description

• Collaborate with a multidisciplinary team of care providers to deliver coordinated, whole-person care. • Develop individualized care plans that may incorporate nutrition, behavioral health, supplementation, lifestyle modification, medication management, and other evidence-based interventions. • Evaluate patient symptoms and clinical progress, order and interpret laboratory testing when appropriate, and collaborate with supervising physicians on treatment planning and escalation of care. • Provide longitudinal support to patients through telehealth visits and asynchronous communication, helping patients implement care plans and achieve their health goals. • Adhere to evidence-based clinical guidelines and best practices, including those established by the American Gastroenterological Association (AGA) and American College of Gastroenterology (ACG). • Deliver care virtually through Ayble's telehealth platform while maintaining high standards of clinical quality, patient engagement, and documentation.

🎯 Requirements

• Master of Science in Nursing (MSN), Post-Master's Certificate, or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program. • Current national board certification as a Nurse Practitioner through ANCC, AANPCB, or another recognized certifying body. • Active, unrestricted RN and APRN licenses in at least five (5) U.S. states. • Ability to commit to a minimum of 30 hours per week. • Experience working collaboratively within a multidisciplinary care team. • Experience supporting patients with chronic disease management, behavior change, and longitudinal care. • Familiarity with evidence-based communication strategies, including Motivational Interviewing and other behavior change techniques. • Comfort using telehealth platforms, electronic health records, and digital collaboration tools. • Demonstrated empathy and a patient-centered approach to care. • Ability to successfully pass background, credentialing, and reference checks.
