📋 Description

• Conduct initiating telehealth visits, including E/M or Annual Wellness Visits, to establish medical necessity for care navigation services • Identify high-risk conditions and unmet social needs • Document findings in Baba’s platform, including accurate coding, SDOH Z-codes, diagnoses, and risk factors • Develop and approve individualized care plans aligned with patients’ medical treatment plans • Collaborate with care advocates, nurse care managers, and clinical operations to finalize care plans and enroll patients safely and compliantly

🎯 Requirements

• Completion of an accredited Nurse Practitioner program (required) • Active Nurse Practitioner or APRN license in good standing, with licensure in multiple states (required) • 2+ years of experience in primary care, telehealth, or care management (required) • Familiarity with Medicare programs, care navigation, incident-to requirements, and SDOH documentation • Proficiency with EMRs, care management platforms, and telehealth workflows • Multiple active state Nurse Practitioner licenses • Legal authorization to work in the US • Bilingual ability is a plus
