📋 Description

• Provide compassionate, patient-centered personal care to hospice patients in their homes and community settings • Assist with activities of daily living, including bathing, grooming, dressing, toileting, and oral hygiene • Assist with safe mobility, transfers, positioning, and ambulation • Observe and promptly report changes in patients' physical condition, behavior, comfort level, or overall well-being to the supervising RN • Provide emotional support, companionship, and reassurance to patients and families • Maintain a clean, safe, and comfortable patient environment, including light housekeeping related to patient care • Follow individualized care plans, infection prevention protocols, safety procedures, and hospice policies • Communicate patient needs, document care provided, and support coordination with the hospice interdisciplinary team

🎯 Requirements

• Home Health Aide or CNA Certification • Valid Driver's License • GED or HiSET, or high school diploma • Basic Life Support certification from the American Heart Association • Certified Home Health Aid certification • Ability to provide hospice care in patients' homes and community settings • Availability for a part-time day schedule, 24 hours, 8a–4:30p • Every 8th weekend requirement
