• Support members’ physical, behavioral, and social health and well-being • Assist members in finding and maintaining competitive, integrated employment • Navigate challenging relationships to ensure members receive necessary services • Perform intake, enrollment, and ongoing assessments • Develop and maintain Person-Centered Plans • Monitor and maintain overall service provision • Partner with members and families to identify supports and direct services for health, employment, and independent or community living goals • Collaborate with clinical and social support colleagues to meet members’ physical, behavioral health, and long-term service needs • Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs • Apply authorization criteria for appropriate home- and community-based services and confirm services are provided and members’ needs are met • Travel to members’ homes in the Montgomery County area
• Must live in Montgomery County or within 25 miles • Available for an 8:00am–5:00pm schedule • RN, or B.S. in social work, nursing, education, human service, health care profession, or related field, or meet federal QDDP/QIDP requirements, or 5+ years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver in lieu of a degree • 2 years of experience in the healthcare industry or an operations-type environment • Valid driver's license • Various immunizations and/or associated medical tests may be required • Completion of digital literacy assessment • Proficiency in Microsoft Office (Outlook, Word, Excel and PowerPoint) • Ability to work independently with minimal supervision or function in a team environment • Problem-solving ability and capacity to manage multiple assignments with critical deadlines • Strong organizational, interpersonal, oral, and written communication skills • Ability to manage multiple projects and tasks with reliability and exceptional work ethic • For routine vehicle use: valid driver's license, compliant auto insurance, and acceptable motor vehicle record • Preference for current CDDN certification for RNs or DDC certification for LPNs • Licensed Master Social Worker or Licensed Clinical Social Worker preferred
• Mileage is reimbursed • Full-time employment • No on-call schedule
