📋 Description

• Provide telephonic triage to determine the most appropriate level of care • Provide requested support and/or virtual monitoring • Offer recommendations, education, and care advice using decision-making tools, clinical judgment, workflows, protocols, and resources • Participate in care coordination to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting • Schedule provider appointments and facilitate provider communication • Ensure accurate and timely documentation in the EMR according to best practices, guidelines, and workflows • Participate in virtual monitoring, reporting, and escalation to identified support services • Support Conduit Health Partners business functions and continuity of operations • Participate in process improvement, professional development, peer development, and peer review • Work evenings, weekend rotations, and three holidays per year • Maintain performance standards prioritizing safety, quality, patient experience, and organizational strategic initiatives

🎯 Requirements

• RN license required in applicable state(s) • Multistate/Compact RN Licensure preferred • ADN or Diploma in Nursing required • BSN preferred • 1 year of acute care nursing experience required • Triage experience preferred • EPIC Electronic Health Record preferred • Ability to provide care appropriate to neonates, infants, pediatrics, adolescents, adults, and geriatrics • Minimum internet speed of 100 Mbps download and 20 Mbps upload at primary and secondary work locations if working remotely • Ability to perform shift work • Required to travel by car to off-site locations occasionally, including in adverse weather conditions • Manual dexterity, finger dexterity, color vision, far and near acuity, and ability to hear alarms, telephone, and audio recordings
