📋 Description

• Provide care and services for a group of patients • Visit patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities • Contribute as a member of the interdisciplinary team • Provide expertise regarding psych-social assessments and interventions • Apply knowledge of community sources • Work in a Case Management, Admissions, or Resource role • Communicate by phone, email, and in writing • Attend meetings with healthcare professionals • Manage visits and documentation • Respond to urgent needs by prioritizing and triaging multiple concerns

🎯 Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership • Work closely with patients on a 1-1 level while receiving support from a patient care team • Ability to work independently on a day-to-day basis • Phone, email, and written communication skills • Ability to attend meetings with a variety of healthcare professionals • Understanding of hospice rules and regulations • Time management skills to manage visits, documentation, and meetings • Ability to respond to urgent needs by prioritizing and triaging multiple concerns • Ability to learn an electronic medical records system • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area
