📋 Description

• Monitor incoming vital signs, symptoms, and health indicators for a panel of patients enrolled in Cadence remote monitoring. • Triage alerts and abnormal readings against established clinical criteria and identify patients who require immediate clinical intervention. • Contact patients with out-of-range readings to gather additional clinical information and confirm symptom, medication, and adherence context. • Coordinate with Cadence nurses and nurse practitioners to route flagged patients into the right intervention and close the loop on every follow-up. • Inspect AI-surfaced screening output and AI-drafted documentation for clinical accuracy before it reaches a provider, correcting anything the model gets wrong. • Document every patient interaction in the clinical record and build care summaries for primary care providers and cardiologists.

🎯 Requirements

• Active Licensed Practical/Vocational Nurse (LPN/LVN) license with compact licensure • Minimum 5 years of experience practicing as an LPN/LVN • Minimum 2 years of experience in a cardiology setting • Experience in remote patient monitoring or telemedicine, including EMR-based clinical documentation • Professional written and verbal communication skills for direct patient contact and provider-facing documentation • Fluency with AI-assisted clinical tools such as Gemini, Open Evidence, or comparable platforms • Ability to explain to providers and patients where AI supports care delivery and where human judgment remains non-negotiable • Availability for scheduled Saturday and Sunday shifts during the stated Eastern Time hours
