📋 Description

• Monitor patient vitals, symptoms, and labs to identify patients in need of clinical interventions • Follow up with patients with abnormal readings to gather more information on their clinical status and triage appropriately • Respond to inbound patient clinical questions and escalations by phone and text message • Lead virtual follow-ups supporting program enrollment, treatment plan changes, medication adherence, and lifestyle goals • Support clinical documentation and develop care summaries for patients' physicians • Ensure every healthcare interaction prioritizes patient well-being and aligns with health-system goals • Support internal growth efforts to scale care delivery for patients with CHF, hypertension, type 2 diabetes, and other chronic conditions • Help shape the culture of a fast-growing Cadence team

🎯 Requirements

• 4+ years of experience treating CHF, hypertension, and diabetes patients in an outpatient or inpatient setting • Compact multi-state RN licensure (RN compact) • Experience in a CHF bridge clinic environment • Experience with remote patient monitoring technology • Ability to monitor patient vitals, symptoms, and labs to identify patients needing clinical interventions • Ability to follow up on abnormal readings and triage appropriately • Ability to lead virtual patient follow-ups supporting enrollment, treatment changes, medication adherence, and lifestyle goals • Passion for patient/customer experience and improving healthcare through digital innovation • Independent thinker/operator
