📋 Description

• Use the nursing process and established protocols to assess patient needs telephonically • Determine the appropriate level of care, provide education, establish plans of care, and communicate follow-up instructions • Escalate and collaborate with on-call providers when additional guidance is needed • Prioritize patient interactions by acuity and need • Apply evidence-based practice to deliver patient care • Implement strategies to reduce patient risk and increase patient safety • Assess patient and family readiness to learn and individualize education • Support shared governance activities and process improvements • Participate in quality and process improvement initiatives • Participate in professional development activities, required meetings, educational programs and annual competencies • Promote collaboration with clinicians and healthcare team members to coordinate patient-centered care • Identify and report patient safety events and near misses and assist with corrective actions • Apply ethical decision-making and participate as an effective member of the patient care team • Assess age-specific patient needs and provide care advice or disposition according to department policies, procedures and protocols • Schedule appointments according to protocol recommendations • Coordinate medical and nursing management, including procedures and medication refills • Maintain and update clinical records according to agency, state and federal guidelines • Document all call encounters in the Electronic Medical Record at the time of the call • Communicate patient status to physicians, Advanced Practice Clinicians and interdisciplinary team members • Provide concise reports on patient responses to care plans • Participate in team meetings and special projects assigned by leadership • Help develop health information resources, system efficiencies and operational resources • Assist with interpreting department policies and advising staff on procedural changes

🎯 Requirements

• Registered Nurse license issued by the state in which the team member practices • Associate's Degree (or equivalent knowledge) in Nursing • 2+ years of acute care experience, preferably within the last 4 years; ED, Urgent Care, ICU and some Med Surg will be considered • Critical thinking skills to independently determine and prioritize patient needs • Knowledge of healthcare specialties, levels of care, symptom identification, and treatment recommendations • Ability to incorporate past experience with established protocols • Excellent verbal communication skills demonstrating empathy, respect, restatement, open-ended questions, active listening and diplomacy • Ability to develop rapport and maintain positive, professional relationships with patients, staff and physicians • Ability to independently organize and prioritize work and manage multiple priorities • Excellent customer service and follow-up skills • Proficiency with computers, internet, desktop software packages and multiple-line telephone systems • Ability to converse with customers/patients while researching and documenting calls on multiple systems • Knowledge of documentation techniques for communication • Required stable and secure internet connection • Functional vision, touch, speech and hearing • Ability to sit for a majority of the workday • Ability to operate all equipment necessary to perform the job • Quiet space to make and receive phone calls • Ability to lift 15 lbs. • Must be available to work every fourth weekend and holiday rotation • Remote work is not permitted from CA, CO, CT, HI, MA, MD, MN, NJ, NY, OR, RI, VT or WA
