📋 Description

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care • Identify and coordinate community resources with patients to support their care • Provide patient education and health literacy on chronic-condition management • Perform medication management, including identifying medication concerns, reconciliation, adherence, and coordinating refills • Assist in ensuring timely delivery of Home Health, DME, Home Infusion, and other critical services • Resolve patient questions and create open dialogue to understand needs • Assist with or manage referrals and appointment scheduling • Work comprehensively with providers to deliver value-based care management initiatives

🎯 Requirements

• At least 3 years of direct patient-facing work experience • Active Minnesota License (LPN) • Experience identifying resources and coordinating needs for chronic care management patients • Technical aptitude with Microsoft Office Suite • Excellent written and verbal communication skills • Active high-speed Wi-Fi • Home office or HIPAA-compliant workspace • Ability to sit for long periods of time • Ability to speak clearly and listen attentively, often by telephone, for extended periods • Ability to understand, process, and take thorough notes in real time on telephone conversations • Epic experience is a plus • Bilingual ability is a plus • Additional state LPN licensures are a plus
