📋 Description

• Work comprehensively with providers to deliver value-based care management initiatives • Manage patient census with a resolution-driven approach to close clinical and non-clinical care gaps • Identify and coordinate community resources for patients • Provide patient education and health literacy on chronic-condition management • Perform medication management, including identifying medication concerns, reconciliation, adherence, and coordinating refills • Help ensure timely delivery of Home Health, DME, Home Infusion, and other critical services • Resolve patient questions and maintain open dialogue to understand needs • Assist with or manage referrals and appointment scheduling

🎯 Requirements

• At least 3 years of direct patient-facing work experience • Active Minnesota LPN license • Experience identifying resources and coordinating needs for chronic care management patients • Technical aptitude with Microsoft Office Suite • Excellent written and verbal communication skills • Active high-speed Wi-Fi • Home office or HIPAA-compliant workspace • Ability to sit for long periods of time • Ability to speak clearly and listen attentively by telephone for extended periods • Ability to understand, process, and take thorough notes in real time on telephone conversations • Epic experience preferred • Bilingual ability preferred • Additional state LPN licensures preferred
