📋 Description

• Collaborate with patients and teammates to engage medically vulnerable or chronically ill populations in their care • Provide support, encouragement, and education to help patients and teammates work toward better health • Communicate with patients, teammates, and caregivers in person, in groups, by telephone, and electronically • Apply coaching, motivational interviewing, behavior modification, goal setting, barrier assessment, and problem solving • Advocate for and facilitate referrals to assistance programs, community-based services, and mental health support • Provide customized, evidence-based education on weight management, exercise, tobacco cessation, stress reduction, and chronic disease self-management • Support shared decision-making with doctors by helping patients understand treatment options, risks, benefits, preferences, and values • Communicate care plans and pertinent information with primary care teams, specialists, and other care participants • Coordinate care across home care, outpatient care, emergency room, and hospital settings

🎯 Requirements

• High School diploma or equivalent required • Bachelor's degree in health discipline preferred • Health Coach Certification required or attained within one to two years of employment • Behavioral Health Experience REQUIRED • Ability to lift and support 50 pounds • Ability to walk, stand, sit, lift, reach, bend, push, and pull • Intact sight, hearing, smell, touch, and finger dexterity • Ability to move about the medical center and travel to various clinical and community locations
