📋 Description

• Assess patient needs telephonically using the nursing process and established protocols • Determine the appropriate level of care and provide patient education, care plans, interventions, and follow-up instructions • Escalate and collaborate with on-call providers when additional guidance is needed • Prioritize patient interactions by acuity and need • Apply evidence-based practice to deliver patient care • Implement strategies to reduce patient risk and increase patient safety • Assess patient and family readiness to learn and individualize education • Support shared governance and process improvement initiatives • Participate in professional development, required meetings, educational programs, and annual competencies • Communicate and collaborate with clinicians and healthcare team members to coordinate patient-centered care • Identify and report patient safety events and near misses and assist with corrective actions • Apply ethical decision-making and participate as an effective member of the patient care team • Schedule appointments according to protocol recommendations • Coordinate medical and nursing management, including procedures and medication refills • Maintain and update clinical records according to agency, state, and federal guidelines • Document call encounters in the Electronic Medical Record • Communicate patient status and care-plan responses to physicians and interdisciplinary team members • Participate in team meetings and special projects assigned by leadership • Contribute to health information resources, system efficiencies, and operational improvements • Assist with interpreting department policies and advising staff on procedural changes

🎯 Requirements

• Registered Nurse license issued by the state in which the team member practices • Associate's Degree (or equivalent knowledge) in Nursing • 2+ years of acute care experience, preferably within the last 4 years; ED, Urgent Care, ICU and some Med Surg will be considered • 2+ years of ED and/or Urgent Care experience highly preferred • Critical thinking and strong problem-solving skills • Knowledge of healthcare specialties, levels of care, symptom identification, treatment recommendations, and established protocols • Excellent verbal communication skills, empathy, active listening, diplomacy, and customer service skills • Ability to organize and prioritize work, manage multiple priorities, and maintain a flexible schedule • Proficiency with computers, internet, desktop software packages, and multiple-line telephone systems • Ability to research and document calls on multiple systems • Required stable and secure internet connection • Must have functional vision, touch, speech, and hearing • Must have quiet space to make and receive phone calls • Ability to lift 15 lbs. • Must be available to work every fourth weekend and holiday rotation
