📋 Description

• Perform comprehensive patient assessments, including OASIS assessments at required intervals • Develop, initiate, and revise individualized plans of care as patient needs evolve • Deliver skilled nursing services and treatments as outlined in the plan of care • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies • Collaborate with the interdisciplinary team to ensure seamless, holistic care • Deliver individualized care to patients in their homes in south Pinellas County

🎯 Requirements

• Graduate of an approved nursing program with an active Florida RN license • Minimum of 1 year of direct patient care experience • Prior home health RN experience strongly preferred • Strong assessment, critical thinking, and patient education skills • Willingness to participate in weekend and holiday on-call rotations • 10+ home health visits per week • Required weekend/holiday on-call rotations
