📋 Description

• Provide remote clinical oversight, telephonic triage, and care coordination • Conduct virtual clinical intakes, health screenings, and wellness checks with patients and caregivers • Perform telephonic triage and clinical assessments for patient calls, directing care using established protocols • Review and update care plans in alignment with physician orders and state regulations • Monitor Remote Patient Monitoring (RPM) data and vital trends, flagging clinical changes for the care team • Serve as a primary virtual point of contact for patients, families, attending physicians, and home care staff • Maintain accurate, timely clinical documentation in EHR platforms • Participate in virtual case conferences, team huddles, and quality assurance reviews • Collaborate with physicians and interdisciplinary teams to ensure quality care and regulatory compliance

🎯 Requirements

• Active, unencumbered Registered Nurse (RN) license in the state of New Jersey required • Multi-state Compact Licensure preferred • Minimum of 1–2 years of clinical experience in home health, triage, or remote care management • Strong clinical triage, assessment, and critical-thinking skills • Professional, empathetic communication skills across phone and video channels • Proficiency with Google Workspace and EHR care management systems • Dedicated home office setup with high-speed internet meeting all HIPAA privacy standards
