📋 Description

• Provide clinical treatment services to targeted populations in an outpatient treatment program • Develop and facilitate assessments and therapeutic interventions, including individual, group, and family therapy • Develop treatment plans and encounter forms for assigned patients and/or providers • Ensure continuity of services through clinical decision-making and compliant documentation, treatment, and case management • Discuss additional service needs with the clinical team • Refer patients to appropriate levels of care, community-based recovery programs, and social or human services • Act as an internal and external liaison with professional services to meet patient care requirements • Participate in assigned external and internal meetings • Participate in program quality assurance and performance improvement activities

🎯 Requirements

• Master’s degree in Psychology, Social Work, Counseling, Education (School Counseling), or related field • CPR – American Heart Association • Act 34 Criminal Background Clearance Certificate • Act 33 Child Abuse Clearance Certificate • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate • Compliance with HIPAA, data security guidelines, the Code of Business Conduct, applicable laws, regulations, company policies, and training requirements
