• Complete and document psychosocial assessments and care coordination • Provide crisis intervention and counseling • Engage in discharge processes with patients and families • Participate in enhancing the role of social work within the hospital
🎯 Requirements
• Required Master's Degree in Social Work • 5 years of Post-graduate experience as a Social Worker • Licensed Social Worker (LSW) OR Licensed Clinical Social Worker (LCSW) • Understanding of the importance of cultural competency • Basic computer skills, including Microsoft Office products • Demonstrated analytical and decision-making skills
